CMR Household Memberships

CMR household membership covers all residents and visitors on the property, and includes any emergency medical treatment and all medical consumables used.

The cost is a highly affordable R119,10 per household per month.

There is also a once-off sign-on and administration fee of R 165.00.

The above membership fees are effective until the 31 March 2012. Membership fees increase on the 1st April each year.

To become a member, simply call CMR’s office on 021 782-0606 and speak to one of our sales people, who can make an appointment to meet with you. If you would prefer to fill out the appropriate forms yourself, please follow the instructions below under “Filling out the Application Forms”.

CMR Business Membership

CMR business membership covers all staff while at work or while travelling in the course of business; and all visitors and customers on the premises. Membership includes any emergency medical treatment and all medical consumables used.

Costs of membership are calculated according to the number of staff, and usual numbers of visitors/customers on the site. A hazardous levy may also apply to businesses that work with hazardous equipment or materials.

Cover for staff and their families at home can also be obtained at a discounted rate if the “Household Add-On” is paid via the company concerned.

To become a member, simply call CMR’s office on 021 782-0606 and arrange for one of our sales people to make an appointment to meet with you at your business premises.

Ambulance Add-On Service Membership

The Ambulance Add-On Service option was introduced to provide a very affordable alternative to paying for emergency ambulance transport if and when needed.

Ambulance Add-on membership is available to CMR household and business members and covers those members listed on the membership for any ambulance transport required from or within the CMR designated area for any emergency medical situation.

Visitors to a members premises are not covered by the Ambulance Add-on service.

Membership includes ambulance transportation as well as any medical consumables used.

Costs are:

• For Household members: R49,50 per household per month. These increase on the 1st April each year.
• For Business members: Quoted after a brief evaluation of the business concerned, but usually less than 50% of their CMR membership fees.
   

Filling out the Application Forms 

1. Household Membership Application Form

• The Primary member (the person who is responsible for payment) should fill this form out. Please complete all sections in block capitals, initial the appropriate spots, and sign it in full where it says “Primary Member Signature”.
• Payment is R119,10 per month for Cape Medical Response membership. If you take the OPTIONAL Ambulance Add-On service, the additional fee is R49,50 per month, thus totaling R168,60 per month.
• Please tick the appropriate box that will indicate whether you want CMR Membership with or without the Ambulance Add-On Service, and initial.
• The once-off admin fee is R165,00. If you are a pensioner, this is reduced to R95,00.
• We prefer a monthly debit order payment.
• If you prefer to pay annually in advance, the annual payment is calculated on the monthly fee/s x 12, less 5%.

2. Members Details Form

• This form is the second page of the CMR Household Membership Application. There is space on the form for the details for two people. Please print an additional page if you have more than three residents, and tick the Subsidiary Member box/es.
• The Primary Member should once again fill out their details here. Please ensure that all additional information such as next of kin, allergies, medication, etc. is completed, as this can be important if you need to be treated.
• Please also ensure that all additional information of Subsidiary Members (all members living with you in the household) is similarly completed.
   

3. CMR Terms & Conditions

A copy of CMR’s Terms & Conditions is available on this website. Please note that these terms and conditions apply to all memberships, and by signing the Membership Application you agree to abide by these terms and conditions.

4. Getting the forms to CMR

The completed forms may be sent back to CMR by fax (fax No 021 782-0707), email (to sales@capemedicalresponse.co.za) or dropped off at the CMR Reception at 106 Kommetjie Road, Fish Hoek during office hours (8am to 4pm).
